
Criteria for Documenting
Medical Disabilities
In order to review your request for reasonable
accommodation, we need to receive the following from your medical care provider:
- Diagnostic statement
- History of problem
- Current status and treatment
protocol
- Prognosis
- Test results
- Challenges presented by disability
- Specific ideas for accommodation
The information must be provided on
the physicians letterhead and signed by your health care provider. The information
submitted must be three years old or less.
